DRV8306: DRV IC failure

We are using the IC DRV8306 & the boost LM5121. Boost section we loaded upto 10A with electronic load. Also DRV we run upto 10A with the motor. Both are working independently. We had tried many times, no failure observed. But soon after integrating both boost output & DRV input , the DRV IC failed at loading 2A. The second unit IC failed in 3A. In DRV IC 3.3V generated from DVDD pin. In failed boards this pin is found to be short to ground. Given below the screenshots of both the schematics and the damaged IC. In damaged IC a black spot is observed. What may be the possible reasons
