DRV8301: Start to report GVDD low error after running for a while

Tool/software:

The DRV8301 chip functioned properly at the beginning and started to show GVDD low error after a few hundard rundowns. Our schematic and layout are below. There are 12 MOSFETs (2 MOSFET in parallel for each high and low side). The MOSFET is BSC010N04LSATMA1. 

C412 is 25v, C405 16v, C408-C410 50v. C403 16v, C404 35V. After reporting the GVDD error, the voltage on GVDD is zero and no voltage was crossing the C412. AVDD and DVDD held the correct voltages. We changed the C412 and C405, same result. The ground Pad is well soldered to the board. I am wondering what kind of failure can kill the charge pump circuit gradually. In our application, we perform the coasting method with turning off all MOSFET when motor is running at high speed. Will it cause potential damage to the bootstrip cap, charge pump cap, or internal charge pump circuit?
