BOOSTXL-DRV8323RH: Gate resistor and DRV8323 damaged

I have replicated BoostXL-DRV8323RH + LaunchXL-F280025C combo as custom board, this's my third time damaged, last 2 was VM and VDRAIN pin separated bit far ... 

This time I can both run on no LOAD and HIGH LOAD, after few seconds HIGH LOAD run, my motor stopped with DRV and gate resistor burned, phase B / VDRAIN / upper Mosfet's D S shorted.

Can anyone help if it's caused by GATE current ? 

Burn on DRV chip location near SPC SNC ?
